Wellness is a shared responsibility, and we hold ourselves accountable to provide the best care possible. In return, we hold you accountable to take an active role in your health. This mutual accountability ensures we make progress on your journey toward optimal function, and ultimately achieve your function goals.

Pre-Appointment Checklist:

• Know your insurance information and bring your insurance cards with you

• Be conscious of body odors: no mud, grease, grime, wear clean clothes or bring a change of clothes, please don’t smoke right before appt

• We have an office dog, Loki, so if you have an allergy, or are uncomfortable with having him present during your appointment, please let us know in advance so we can prepare for your appointment accordingly.

• If you don’t fill out your paperwork in advance of your appointment (you can find new patient paperwork on this page), you must arrive 20-30 minutes prior to your appointment’s start time. The paperwork must be completed prior to starting your appointment.

• If you are under the age of 18, a parent or guardian must be present at all appointments

• As part of your new patient paperwork, please be sure to fill out your insurance information. We can check your benefits prior to your appointment upon request.
